Centre for Health Activation (CHA)
It takes a whole kampung to enable a person to maintain good health and
self-care in the comfort of their homes. As our population ages and the
burden of chronic diseases rises, we recognise the need for close collaborations
with our health and social care partners in the community to enable neighbourhood-based
care to thrive.
Launched in 2017, the Centre for Health Activation (CHA) was
set up to focus on Activation, Research and Training – also known as the ART of CHA.
Its vision is to drive activation and build One Community of Carers (i.e., patients, caregivers, volunteers, health, and social care partners) who are equipped with the skills, knowledge, and confidence to self-care and care for their loved ones and others in the community.

Awareness and Advocacy
Drive health literacy initiatives and develop content for digital platforms and physical spaces for individuals to gain awareness, informational and actionable knowledge to take charge of their own health and the health of others in the community.

Patient-Led National Design Challenge 2025
One of our latest initiatives, the Patient-Led Innovation Challenge, provides a platform for patients and caregivers to co-develop solutions that improve healthcare experiences. By tapping into lived experiences, we foster innovation that is truly patient-centric, enhancing both self-care and community-based healthcare solutions.

Training and Capability Development

Volunteering with Tan Tock Seng Hospital
At the flagship hospital for the Central-North region, we have more than 30 volunteering programmes for individuals to contribute in meaningful ways to support our patients and healthcare workers. Through volunteering, volunteers are trained with essential caregiving skills and become strong advocates to support their community in preventive health measures.
